Core idea

A whitehead is a closed comedone: a small clogged bump beneath the surface. A blackhead is an open comedone: its dark appearance reflects changes in exposed material, not dirt. Forceful squeezing and scrubbing can worsen inflammation and marks.

Look beneath the label

Papules are inflamed raised bumps without visible pus. Pustules have a visible pus-filled center. Nodules and cysts can be deep, painful and scar-forming, so timely dermatologic assessment is preferable to serial cosmetic experiments.

Apply what you learned

Comedones may respond to treatments such as salicylic acid or suitable retinoids when appropriate. Recurrent inflammatory acne may need medical therapy. Choose based on lesion type and severity, not just an 'anti-acne' label, and track new lesions weekly.

Locate the sebaceous gland and its connection to the follicle. Oil production differs from outer-skin water; shine and temporary dehydration can coexist.

Blackheads are not dirt

A blackhead is an open clogged follicle whose exposed contents darken, not proof of poor hygiene. The nose also has normal sebaceous filaments that can resemble tiny dots; these are functional structures, not necessarily acne requiring aggressive extraction. Distinguishing them prevents unnecessary harsh treatment.

Deep acne needs a different response

Deep painful lesions can scar even without a visible surface head, and home piercing or squeezing can worsen inflammation. Comedones often call for a different topical approach. Match management to lesion type and severity instead of treating every bump identically.
